Step 3: Completing the Application Form

The application form you need to complete is an FL401. This form can be either collected from any County Court, or downloaded now by clicking on the red button below (Please note that you will need Adobe Reader to view the file).

Once you have downloaded the form, you will notice that the last three pages are notes on how to complete the form. It should be fairly self explanatory, but we will run through it with you below.

Box 1: Your name, address and date of birth. Where it asks for your solicitor's details, write "Litigant in Person"

Box 2: The Respondent's (your ex-partner) name, address and date of birth

Box 3: Tick the top and bottom box, leaving the middle one empty

Box 4: Tick the box that describes your relationship

Boxes 5, 6, 7 & 8: Leave blank

Box 9: Complete only if you require an interpreter in court

Box 10: Give names and dates of birth of any children living at your address

Box 11: Only complete this if you are involved in any other Family Court proceedings

You will need to take three copies to Court with you, so once you have downloaded it you either need to print off and complete three copies, or make additional photocopies of your completed one.
